The CBS Gene Variant, Explained

CBS is one of the most mythologized entries on any methylation report. Online, it’s often tied to dramatic claims about sulfur, ammonia, and elaborate diets. The reality is more measured — and it’s worth separating the common variant most people carry from the rare, serious mutations that share the gene’s name. Here’s the honest picture.

What the CBS gene does

CBS stands for cystathionine beta-synthase. Its enzyme sits at a branch point in the methylation cycle: it directs the amino acid homocysteine down the “transsulfuration” pathway, which eventually contributes to compounds like cysteine, glutathione, and taurine. In short, CBS helps route homocysteine toward being used up rather than recycled.

Two very different things called “CBS”

This is the key distinction the internet usually blurs:

  • Rare, pathogenic CBS mutations can cause homocystinuria — a serious inherited metabolic disorder that is diagnosed and managed clinically. This is real, but uncommon, and not what a consumer methylation report is flagging.
  • Common CBS variants (like C699T / rs234706, or A360A / rs1801181) are the ones that appear on consumer reports. These are common, and their health significance is modest and far less clear.

Confusing the two is where a lot of online fear comes from. Carrying a common CBS variant is not the same as having homocystinuria.

The “upregulation” claims — be skeptical

A popular online narrative holds that common CBS variants cause the enzyme to “run too fast,” draining the cycle and driving sulfur or ammonia problems — often followed by specific restrictive diets or supplement protocols. The evidence for this specific narrative is weak and contested; it’s not an established clinical finding. Be cautious with content that presents it as settled fact, especially when it leads to a product or a protocol.

What the research reasonably supports

The defensible summary: CBS is a real gene with a real role in the transsulfuration pathway; rare mutations cause a serious disease; and the common variants on consumer reports are common, with modest and uncertain individual significance. Any associations are relative and population-level, not personal predictions.

How to find your result

Your existing 23andMe or AncestryDNA file already contains these markers (look up rs234706, for example). As with other variants, raw files can report a different strand than papers use, so an analysis tool that reports your genotype directly is less error-prone than manual reading.

What your result means — and what to do

If you carry a common CBS variant:

  • It’s common and, on its own, not a diagnosis — and certainly not the same as homocystinuria.
  • Treat dramatic online claims and the diets built on them with skepticism.
  • If you have real symptoms or concerns, the right path is a qualified practitioner who can order appropriate testing — not a self-directed protocol from a forum. We deliberately don’t recommend diets, supplements, or dosing here.
